Use this form for each student at the beginning and end of each period that the We Choose Virtues system is used.
You may choose to circle the appropriate number value in two different colors so you can compare the results on the same form. After the Virtues have been introduced, the teacher and student may want to work together to establish “target numbers” to help motivate the student toward personal change, as well as communicate expectations to parents.
